說明：
一、依據社團法人台灣癲癇之友協會 107 年 9 月 12 日(107)台瑞字第 016 號函辦理。
二、該協會為獎勵在學的癲癇朋友，能不畏懼疾病的困擾，努力完成學業，特設此項獎、助學金。
三、符合資格且欲申請旨揭獎、助學金者，請自行至該協會網站(網址： http://www.epilepsyorg.org.tw)下載相關表格，填妥並備齊相關文件後，於 107 年 10 月 15 日(星期一)前郵寄至社團法人台灣癲癇之友協會(地址： 10549 臺北市敦化北路 155 巷 66 弄 41 號 B1)，以郵戳為憑。
四、經審核通過者，由該協會個別通知並公告於該協會網站，並於 108 年 1 月 26 日年會上公開頒獎表揚。
