一、依據教育部 111 年 1 月 7 日臺教師（三）字第 1112600011 號函暨「教育奉獻獎選拔及表揚要點」（附件 1 ）辦理。
二、「教育奉獻獎選拔及表揚要點」（以下簡稱本要點）相關規定略以：
(一)第 2 點：以公私立學校已退休或離職之非現職校長、教師及軍訓教官為選拔表揚對象。
(二)第 3 點：教育奉獻獎之推薦條件，應具有投入教育或學生輔導之志願服務相關工作事蹟。
(三)第 4 點：各機關學校及經許可設立之民間團體組織或基金會，於每年二月一日至三月三十一日前，向本局提出推薦。本局至多提報 5 名至教育部複審。
(四)第 6 點：最近三年內曾獲本要點或教育部其他相關規定獎勵者，不再受理推薦或給獎。
(五)第 7 點之 1 ：經遴薦獲獎人員，如有不實或舛錯者，應撤銷其資格，並追繳獎勵金及相關獎勵。
三、有關受推薦人員品德操守部分，請於提出推薦時確實查證，並於旨揭期限前寄達下列資料（含電子檔光碟）至本局社會教育科（ 708201 臺南市安平區永華路二段 6 號 4 樓）：
(一)111 年第 17 屆教育奉獻獎推薦名冊（受推薦人員如有 2 名以上，請於名冊註明優先順序）（附件 2 ） 1 份。
(二)111 年第 17 屆教育奉獻獎受推薦人具體事蹟表含佐證資料（請詳閱填表說明，依規定填寫，須簽章）（附件 3 ） 1 份。
(三)近半年「彩色大頭照」電子檔（檔案格式 JPG 、檔案大小 800KB 至 4MB ） 1 張。
