說明：
一、依據本府衛生局 111 年 6 月 13 日南市衛疾字第 1110093765 號函辦理。
二、 V-Watch 自本(111)年 5 月 3 日起已將小於 18 歲的兒童及青少年納入追蹤監測對象，經由父母或監護人同意並透過手機代為填寫健康回報，以積極加強監測、收集分析該族群接種後常見不良反應發生情形，提升兒童、青少年 COVID-19 疫苗安全性監測，以及相關關懷照護與因應處置效率。
三、為加速宣傳、鼓勵家長代孩童加入 V-Watch ，提升監測效率，請貴校（國小-接種基礎劑莫德納、 Pfizer-BioNTech 第一、二劑、高中及國中-接種追加劑)協助發放宣傳文宣（如附件， A4 大小，內含可掃描加入之 QR code ），並請提醒可以擇下列方式之一加入：
(一)於校園接種現場留觀區發送 V-Watch 單張給陪同家長，於當場掃描 QR code 加入。
(二)由班導師於班級統一分發（可併入聯絡簿） V-Watch 單張，由學生帶回給家長掃描並加入。
(三)亦可將 V-Watch 單張掃描/拍照後，請班導師轉傳班級 LINE 群組，提供家長以電腦、手機掃描附件 1 之 QR code 加入，並提醒必須於接種 COVID-19 疫苗後三日內完成掃描及註冊，若未接種請勿掃描，避免資料無法比對，致後續無法再加入。
(四)另本府衛生局亦已印製 V-Watch 海報提供接種院所張貼使用，於接種留觀區域供家長參用加入。
四、另請貴校加強衛教，無論加入 V-Watch 與否，若接種後出現身體不適或任何健康問題，請務必到醫療院所就醫，及時診療，釐清原因。
