一、依據教育部 103 年 5 月 27 日臺教社(三)字第 1030074560 號函辦理。
二、旨揭實施要點及推薦表格（甲、乙表）可至教育部網站（http://www.edu.tw）/認識教育部/本部各單位/終身教育
司/電子佈告欄下載。
三、凡曾接受全國性推展社會教育有功團體及個人表揚之團體或個人，五年內不再受理推薦。
四、推薦表 (如甲、乙表) 之內容，請依下列規定填寫，以利評審：
(一)顯著事蹟欄，以近三年內之重要事蹟為主，依事蹟發生先後，條列敘明。
(二)推薦單位應填妥相關資料，於推薦單位欄加蓋印信，並備妥有關證明書文件及活動照片等資料供參。
(三)推薦短文欄，應撰寫六百至八百字之短文，綜合簡介被推薦者之事蹟及貢獻，並附標題。
五、推薦表及申請推薦繳交之證明書文件等資料，不予退還。
