一、依「教育奉獻獎選拔及表揚要點（以下簡稱要點）」第 3 點規定，教育奉獻獎之推薦條件，應具有投入教育或學生輔導之志願服務相關工作事蹟，著有具體績效，足為典範者。
二、復依要點第 4 點規定，各機關學校及經許可設立之民間團體組織或基金會，於每年 2 月 1 日至 3 月 31 日前向本局提出推薦，本局至多推薦 5 名至教育部參與選拔。
三、另依要點第 5 點及第 6 點規定，第 13 屆教育奉獻獎表揚人數以不超過 20 名為原則，最近 3 年內曾獲本要點或教育部其他相關規定獎勵者，不再受理推薦或給獎。
四、有關推薦人員之品德素行部分，請貴單位於初審時確實查證。經遴薦獲獎人員，如有不實或舛錯者，應撤銷其資格，並追繳獎勵金及相關獎勵。
五、因應教育部使用網站上傳推薦資料，所附推薦名冊等各項表件注意事項：
(一)請務必詳閱填表說明，依格式、字數限制填寫，格式有誤者一律退件。
(二)推薦單位各項欄位請詳填，推薦理由無字數限制，請務必填寫。
(三)相關佐證請擇要掃描成 PDF 檔，可上傳 10 個佐證資料，每個佐證資料限 20MB 以內，檔名請簡述佐證資料內容。
(四)請提供近半年「正面個人獨照」電子檔 1 張（身體比例至少占照片 1 半，半身照為宜， JPG 格式、 800K-4MB 、直式）。
