二、旨揭活動因投稿案件數較少，故延期至 113 年 10 月 30 日（星期三）截止，並新增郵寄、親送及團體等報名投稿方式。
三、報名方式：將著作權同意書、心得及報名表以下列方式送達。
(一)網路報名：至 https://p.tainan.gov.tw//j4ToB1 填寫報名。
(二)郵寄報名：郵寄至衛生局林森辦公室國民健康科林佳蓉衛生教育指導員收，以郵戳為憑。
(三)親送報名：親送至衛生局林森辦公室國民健康科(2)。
(四)團體報名：由學校、課程照顧中心、或課後才藝班統一報名，提供聯絡窗姓名、電話及信箱，以郵寄、親送、 e-mail(信箱： a00122@tncghb.gov.tw ，請務必來電確認是否已收到)。
四、檢附活動計畫書、著作權授權同意書、心得及報名表及活動單張（如附件）。
