轉知-本市114年度國中教師市內介聘積分審查作業注意事項

⼀、依據本市 114 年度市⽴中等學校教師市內介聘分發期程表辦理。

⼆、為避免出席⼈員等候多時，請參加審查者配合下列事項：

(⼀)分時段進⾏書⾯審查，請申請介聘教師依排定時間出席積分審查(詳如附件 1)，若無法於排定時間前往，請⾃⾏擇時前往，並請耐⼼等候。

(⼆)委託他⼈參加積分審查者，當⽇應另檢附「積分審查委託書」 (如附件 2)及雙⽅⾝分證正本。

三、申請市內介聘之教師積分審查注意事項如下：

(⼀)審查地點：中山國中中山館。

(⼆)審查時間： 114 年5⽉1⽇(星期四)上午 9 時⾄中午 12 時，下午 1 時 30 分⾄下午 3 時【逾時不予受理】。補件時間為下午 3 時⾄下午 4 時。

(三)審查當⽇應備⽂件：申請表(請以A3 格式列印)、教師證書、學歷證明⽂件、積分證明相關⽂件，上述各項證件正本及影本各 1 份依序排列裝訂成冊(除申請表、服務年資及研習證件正本由本市教師介聘甄選分發⼩組存查外，其餘證件正本驗後當場發還，影本由⼩組存查，另影本請申請⼈簽章並註明與正本相符)。

(四)未於教師介聘系統登錄者，不予受理現場報名。

提醒：參加國中市內介聘(含超額介聘)之教師請於114 年 4 月 21 日(星期一)上午 8 時起至 114 年 4 月 28 日(星期一)中午 12 時止，至 114 年度國民中小學暨幼兒園教師市內介聘系統填報基本資料及積分。( https://match.tn.edu.tw/TP/ )

(五)若教師係經「超額介聘」、「學校合併或停辦之介聘」、「因新設學校致學區重劃之學校教師介聘」⾄貴校服務，或教師於校內曾有轉任情形，請學校⼈事⼈員應於服務證明書中加註。

(六)證件不⿑須補件者，請於 114 年 5⽉ 1⽇ (星期四)下午 4 時前送達中⼭國中中⼭館，逾時視為放棄。

四、參加超額介聘之教師，請貴校惠予公假並課務排代；參加⼀般介聘及⾃願赴偏遠地區學校服務介聘之教師者，則惠予公假惟課務⾃理。

五、中⼭國中不提供停⾞空間，請多利⽤周邊道路停⾞，勿占⽤店家⾨⼝以免受罰。

六、檢附「本市 114 年度市內介聘積分審查補充說明」 (如附件 3)及「服務證明書格式(範例)」 (如附件 4)各 1 份，請各校⼈事⼈員參考格式開具服務證明書(內容請務必含該師係經 000 學年度市內介聘/市外介聘/教甄錄取⾄貴校)，俾落實審查機制。
